Completed Enrollment Packets
Bring the following items to Friendship House:
- Completed enrollment packet.
- $30 Registration Fee per family for preschool.
- Child(s) Immunization Record
- Copy of your child’s insurance/Medicaid card

REgistration Packets
To ensure enrollment at Friendship House, every student must submit a completed registration packet. You can download the PDF below and either email it to programs@friendshipmt.org or drop off the completed package at 3123 8th Ave South between 7:30 am and 5:30 pm.
Youth enrichment programs are based on sliding fee scales. Please inquire about Best Beginnings or FH Scholarships for qualifying families.

Join Our Teen Program
Friendship Teens
Join us in creating a fun and safe place for teens to hang out. Through Friendship Teens, we aim to be a beacon of HOPE and community support for the teens in our neighborhood by reflecting the value of kindness.
Friendship Café: Fridays, every 6:00pm – 8:00pm (follow Instagram for updates)
Waiver required before participating in our Friendship Teens activities.
