TEEN PROGRAM WAIVER AND RELEASE

• Assumption of Risk: participation in Friendship Teen Events may involve various recreational and physical activities. I acknowledge that these activities inherently carry certain risks of personal injury, sickness, death, and property damage or theft. I expressly assume all risks associated with my child's participation in these activities.
• Release and Hold Harmless: I release, forever discharge, and agree to hold harmless Friendship House of Christian Service and Friendship Teens, along with its officers, directors, members, agents, servants, and employees, from any and all liability, claims, or demands arising from personal injury, sickness, death, or property damage or theft incurred during my child's participation in the described activities.
• Medical Authorization: I authorize Friendship House of Christian Service and Friendship Teens to seek medical treatment, including emergency surgery or medical treatment, for my child if deemed necessary by a medical professional, including first responders. I assume responsibility for all medical bills incurred at the time of the incident and all future medical issues.
• Emergency Contact Information: I will provide accurate and updated emergency contact information for my child, including any relevant medical conditions or allergies.
• Photography and Media Release: I grant permission for Friendship House of Christian Service and Friendship Teens to capture and use photographs or videos of my child for promotional or educational purposes related to the teen program.
• Authorization and Permission are hereby given to Friendship House of Christian Service and Friendship Teens to furnish any necessary transportation, food, and lodging for this participant.
• Indemnification: I agree to indemnify and hold harmless Friendship House of Christian Service and Friendship Teens for any liability resulting from my child's negligent, willful, or intentional acts.
• Choice of Law and Venue: This waiver is governed by the laws of the state of Montana. Any legal action arising from this waiver shall be brought in the courts located within Montana.
